Pulse diagnosis is used in conjunction with other methods of diagnosis such as inspecting –making a general physical observation of the patient; auscultation and olfaction – listening and smelling as well as questioning – obtaining general information about the patient’s symptoms.

Over the past centuries, pulse diagnosis has been in practice by physicians in both Japan and China. It is true that the intricacies of pulse diagnosis are difficult to master and Western physicians regard the process as subjective. However, the fact that it is a salient diagnostic tool for TCM practitioners and patients cannot be denied.

Why is Pulse taken in Traditional Chinese Medicine?

Pulse diagnosis in Traditional Chinese Medicine (TCM) serves several purposes, primarily assessing the condition of the patient’s blood and Qi, or vital energy.

According to TCM principles, Qi flows through the body’s acupuncture meridians, influencing overall health. Pulse diagnosis helps acupuncturists detect irregular Qi movements in specific areas of the body, aiding in the assessment of organ health.

Pulse Examination

Throughout medical history, various body locations have been utilized for pulse diagnosis. Initially, nine locations were examined: three on the head, three on the hand, and three on the legs. Some practitioners still use these along with other pressure points.

Modern practice often simplifies this, focusing on the radial artery above the wrist. Practitioners assess three finger positions (cun, guan, and chi) and apply three levels of pressure (superficial, intermediate, and deep) to feel the pulse. Both wrists are checked, and the readings are used to categorize the pulse.

Types of Pulses and Their Meanings

There is no fixed number of pulses that can be taken, with different sources suggesting anywhere from 12 to 40 types. However, a consensus among practitioners recognizes 24 distinct pulses:

These pulses are broadly grouped into categories. A rapid pulse, defined as 5 beats per breath, can be further classified as forceful or weak, indicating excess or deficiency of Qi. Similarly, a slow pulse can also be categorized as forceful or weak. Other broad categories include floating, deep, slippery, rough, irregular, and bowstring pulses.

Precautions and Considerations

For accurate results, it’s essential to perform a pulse diagnosis on a patient in a normal state. To ensure accuracy, avoid performing pulse diagnosis on patients immediately after eating, drinking, exercising, engaging in sexual activity, or in extreme temperatures.

Both the practitioner and the patient should be in a relaxed state with normal breathing before conducting a pulse diagnosis.

While pulse diagnosis is valuable, it’s best used in conjunction with other diagnostic methods, especially for patients with chronic or severe conditions or when the underlying cause of a disorder is unknown. Employing multiple diagnostic methods can lead to a more precise diagnosis.
